This 3 day course is designed to deliver the foundation of knowledge needed to conduct the assessment of the dizzy patient.Through theory and practical workshops the performance of vestibular assessments will be applied to clinical practice. On completion participants will have an understanding of vestibular disorders, there descriptions and understand the technologies to locate, identify recognise vestibular dysfunction. The emphasis of this course is on small group work to ensure maximum learning opportunities for all participants.

Day 1

Building a foundation through knowledge
Intended Learning Outcomes

  • Describe the anatomy & physiology of the vestibular / balance system
  • Explore how patients reported symptoms can fit with patterns of pathology
  • Discuss & demonstrate how to perform bedside assessments to inform the assessment strategy

Morning session

  • Introduction to the ‘Dizzy Patient’: The art of taking a ‘balance’ history
  • Anatomy & Physiology of the Vestibular / Balance system

Afternoon session

  • Clinical assessment of the Dizzy patient form an ENT perspective
  • Does Neurology look for additional things when clinically assessing the Dizzy patient?
  • Practical Workshops:
  • Workshop 1: Bedside assessment (eye movements, gait, positional and positioning including Dix-Hallpike and Sidelying, HIT etc…
  • Workshop 2: Technology at the bedside, vHIT (introduction) and CTSIB
  • Q&A with the faculty

Day 2

Assessment of the 5 vestibular end organs
Intended Learning Outcomes

  • Understand the theory and application of objective measurement of the vestibular organ
  • Perform assessment of semi-circular function using vestibular ocular reflexes for objective measurement
  • Perform assessment of otolith function using cervical and ocular Vestibular Evoked Myogenic Potentials
  • To discuss and apply the findings of vHIT and VEMP in the context of quantifying vestibular function

Morning session

  • Introduction to vHIT and theoretical background
  • vHIT artefacts and how to overcome them
  • Group breakout:
  • vHIT hands on Workshops
  • vHIT, hands on, tips and tricks
  • Q&A Session

Afternoon session

  • Otolith organ assessment (Vestibular Evoked Myogenic Potentials)
  • Introduction to VEMP, theoretical background, & clinical utility
  • VEMP, hands on, tips and tricks
  • Workshop 1 – cVEMP
  • Hands on demonstration and (see and do) workshop to perform Cervical VEMP
  • Workshop 2 – oVEMP
  • Hands on demonstration and (see and do) workshop to perform Ocular VEMP
  • Clinical application – How to put the 5-end organ picture together
  • Q&A Session

Day 3

Interpreting eye movements – Videonystagmography
Intended Learning Outcomes

  • Understand the theory and application of objective measurement of eye movements when assessing the dizzy or imbalanced patient
  • To measure and perform eye recordings in gaze, movement, and ear specific testing through Videonystagmography
  • To discuss and apply the findings of vestibular assessments in the context of quantifying vestibular function, identifying the identity of dysfunction, and gaining clarity in reporting findings

Morning session

  • The Clinical Pathway – What is the question? Experience from evolving clinical practice
  • Calibration Gaze Testing, and Oculomotor Examination
  • Positional / Positioning Testing & associated nystagmus
  • Interactive session / quiz – video eye movement interpretation

Afternoon session

  • Caloric testing – Ear specific testing
  • Background revisited, how to interpret, and clinical considerations
  • Hands on workshop – Performing the caloric test
  • Clinical application from a Case Example perspective – Reporting on test findings & bringing them together when making a report
  • What is the future of dizziness assessment in the UK?
